Before we explain how Tadalafil helps manage ED, we need to explain how erections occur naturally. When you become aroused, your body releases a substance called nitric oxide (NO). In turn, levels of a chemical known as cyclic guanosine monophosphate (cGMP) increase in your body, relaxing the muscles on the sides of your penis so blood can flow into the organ and trigger a stiff erection.

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. Whether you can take Tadalafil at the same time as your other medications will depend on which medications you are taking.
You are required to complete a short medical consultation questionnaire that asks for any medications that you are prescribed from your GP and will indicate the medications that can’t be taken alongside Tadalafil.
There are several medicines which should not be taken concurrently with Tadalafil. Medicines which can have potentially serious interactions with Tadalafil include :-
• Any alpha-blocker medicines e.g. doxazosin (Cardura), alfuzosin (Xatral), tamsulosin, terazosin, or Flomax, that you can buy in the pharmacy. (These medicines may be used to treat prostate problems or high blood pressure).
• Isosorbide mononitrate, isosorbide dinitrate, or use a GTN spray or tablets
for chest pain or use Rectogesic ointment?
• Nicorandil (Ikorel), used to treat angina
• Cimetidine, used to treat stomach or duodenal (peptic) ulcers
• Theophylline, used to treat asthma
• Antibiotics:- Clarithromycin, Erythromycin and Telithromycin
• Anti-fungals such as itraconazole and ketoconazole
• Seizure medication:- carbamazepine, oxcarbazepine, phenobarbital, phenytoin and primidone
• Heart or blood pressure medication:- disopyramide, amiodarone, quinidine and procainamide
• Anti-viral medication:- telaprevir, atazanavir, retonavir, indinavir, saquinavir and cobicistat
• Recreational drugs:- amyl nitrate (poppers)

This is a single dose study with a single man and his partner. The study includes four doses of Tadalafil and four doses of Cialis. Four men were treated with four different medications. The four treatments were prescribed by a single doctor (Boehringer Ingelheim, New York, New Jersey, USA).
Two of the men were treated with Cialis and one with Tadalafil were treated with one medication. The other four men were treated with one of the Tadalafil medications. After a median time of 4.4 days was specified, these four treatment groups were equally effective (AOR (95% CI) 0.99 (0.97, 0. peak). The effect of treatment with Tadalafil was similar for both the treatment groups. There was no significant difference between treatment groups for any other measurements.
Tadalafil was taken in the morning and the men were followed up every 2-3 months, every 6 months for a median time of 3.4 years. The average time taken to stop taking Tadalafil was 3.4 years in the Cialis treatment group and 2.8 years in the Tadalafil treatment group. There were no cases of priapism (prolonged erection) during this period.
Priapism was seen in 4 of the men treated with Cialis (2.8%), Tadalafil (2.8%) and Tadalafil (1.4%). In one of these cases, the men developed facial flushing and in the next, they developed an erection. There were no cases of priapism during this period. The men were not reported to have had any problems with sexual intercourse with their partner.
